Health Department Seeking Primary Care Practices to Implement Strategies for Blood Pressure and Diabetes Management

LEONARDTOWN, MD (September 15, 2015) – The St. Mary’s County Health Department is seeking adult primary care practices (providers) to implement clinical quality improvement strategies for blood pressure and diabetes management. The Addressing Chronic Diseases in Health Systems Initiative will focus on implementation of team-based care strategies for blood pressure and diabetes control, as well as the use of community-based resources to assist in optimizing patients’ health. Funding for this initiative is provided through the Maryland Department of Health and Mental Hygiene.
